Here are a few things to take note of:

▪️ To book an appointment, you will need to create a profile on this platform.

▪️ Please make sure to click “Intake Forms” in your client profile to ensure all necessary forms have been completed.

▪️ We require a credit card or debit/credit card on file to reserve your session time. Please add to your profile/account.

Direct Billing

We will make every effort to direct bill for your visit, as long as your insurer allows and all information is correct. However, clients are fully responsible for all costs in the event that we cannot direct bill your insurer and you will be issued a receipt so that you can submit your claim yourself (which many insurers make very easy to do on-line).

Cancelation Policy

Cancellations with less than 24 hours’ notice will be charged the full session fee. Any cancellations between 25 and 48 hours will be charged 25% of the session fee.
